* If the Resident elects to make an appointment for a specific time and date then fails to be present, then the Resident expressly agrees to pay a fee of $40.00 to the management as additional rent before the scheduled work order will completed.

Normal service requests are handled at no charge to our residents, however, if a work order or a service call is made regarding damaged property which was caused by the carelessness, misuse or neglect of the Resident(s), his/her family, or visitors, or pets, the Resident(s) hereby agrees to pay a service call fee of $20.00 per visit, in addition to a labor charge of $25.00 per hour for the work that was done.

I understand that by submission of this service request I hereby authorize property personnel to enter my apartment to perform the above noted repairs. Emergency repairs as deemed necessary by management must and will be completed as soon as possible, for non-emergency repairs please select one of the above noted options.
